COVID-19 CONSENT
Please read this rules and policy before you use us. We want
to do it right at the first time. Your satisfaction is what Angel Wellness LLC. needs.
Reservations and Cancellation
A major credit card or debit card is required to make your
reservation. If you are not able to keep your appointment, you must
provide a minimum of 5-hours' notice to avoid being charged the
full price of your missed treatments.
For Minor
If you are under the age of 17, you must have a consent form signed
by a parent or guardian.
Arrival Time
Please arrive at least 15 minutes prior to your scheduled
appointment so you can make full use of our spa facilities.
To maintain the full massage hour we ask you silence your cell
phones devices upon arrival. Regretfully untimely arrivals may
be subject to reduced treatment time to remain
on schedule for our next guests.
Personal Value
For your convenience, Please put your personal belongings in our
safe place box. However, we suggest you keep your valuables
at home. Angel Spa will not be responsible for lost or stolen items.
Services & Prices
Services and prices vary by times and days. We reserve the right to
modify, discontinue or change services and prices without notice to
ensure that higher standards of service and quality are being met.
Intake and health history form
Clients with medical conditions are advised to consult their doctors
prior to booking any spa services.
Please be kind and let us know any existing medical
conditions by completing health history intake form prior to receiving
your treatments. Angel Spa reserves the right to discontinue a service
for any reason to ensure the safety of all parties.
Right to refuse the service
Angel Wellness LLC reserves our right to refuse the service.
